What is the definition of Short Term Disability (STD)
Disability means that due to sickness, pregnancy or accidental injury you are limited from performing the material and substantial duties of your own occupation. You are unable to earn more than 80% of your pre-disability earnings at your own occupation for Cummins Inc. or for any employer in your local economy and are receiving appropriate care and treatment from a doctor on a continuing basis. Your loss of work must be a direct result of your sickness, pregnancy, or accidental injury.

How do I know if I am eligible for the STD benefit?

  1. You are an active employee of Cummins Inc. and
  2. You work a minimum of 20 hours per week

This is a company paid benefit – no employee contributions are needed to participate in this Plan.

What does the benefit cover?
If you have a serious health condition such as illness, accidental injury, pregnancy or hospitalization, that limits you from performing the material and substantial duties of your job, you may request Short-Term Disability (STD) benefits through Unum, our third-party administrator.

If you expect your absence due to sickness or injury to exceed 3 consecutive calendar days (called the elimination or waiting period), you must contact Unum to file a claim. During the elimination period, you may choose to use sick time to continue pay.  For planned leaves, such as a scheduled procedure, pregnancy etc., claims must be reported no earlier than 30 calendar days prior to any known absence, and no later than 30 calendar days after the initial date of absence.

If approved by Unum, the plan will replace 100% of your income for up to 3 months, then 75% of your income for the next 3 months*. If your disability continues beyond 6 months, Unum will provide details on the Long-Term Disability requirements and next steps.

When do I report a disability?
You should report a disability to Unum:

  • If your physician has determined that you are unable to work due to illness, injury or maternity reason, or
  • Thirty days in advance of a planned medical absence, such as prescheduled surgery or an expected maternity leave.

If you are unable to call because of an accident or other medical emergency, your spouse or other family member can make the call instead. If they can provide at least your name, the nature of your disability and the name and phone number of the hospital or other treatment facility, Unum can get started on the claim.

How do I begin the process?
Filing for Short Term Disability is quick and easy. For new claims, Call Unum at 1-866-229-4885 and Press 1. This is a phone line just for Cummins employees and is staffed from 8 a.m. to 8 p.m. Eastern Time.

When you file your claim with Unum, the following information will be validated and/or requested:

  • Name and Social Security Number or employee ID number
  • Date of birth and marital status
  • Complete address and phone number
  • Company name and occupation (job title)
  • Physician’s name, fax and phone numbers
  • Brief description of medical condition (illness or injury)
  • Cause of medical condition (illness or injury) or need for leave
  • The last day worked and first day absent from work due to condition
  • Date of the first visit, the most recent visit and the next scheduled visit with your physician for this condition
  • Date you expect to return to work (if known) or the actual date, if already returned to work

You will be connected to a Unum Intake Specialist to assist you in applying for STD. Unum will mail your application to your home within two business days. If you go to www.unum.com or download the UNUM phone app, you may also receive e-communication about your claim. Make sure you have a signed authorization form on file with your doctor. This will help Unum obtain your medical information faster.

You must fill out your sections of the claim form and then give it to your attending physician. Your doctor will need to complete the Attending Physician Statement and you or your doctor must fax the completed documentation to Unum at 800-447-2498 as soon as possible. Any delays with the required paperwork could impact your pay while out on short term disability.

If Unum has not received the required information within 45 days from the date you called Unum, your application will be denied. Upon Unum receiving a completed application, you will generate an approval or denial letter within five days. Continue to follow your location’s call-in procedure until you have been approved for leave.

Are STD eligibility and approval the same thing?
No, you are eligible if you are an active employee working a minimum of 20 hours per week. Approval means that you have submitted the completed medical documentation and your condition meets the definition of disability.

Long-Term Disability (Applicable to Senior Hires Only)
If you were hired before June 19, 1990 (Senior Hires), you may be eligible for a long-term disability (LTD) benefit of 60% of your base pay (up to a monthly maximum of $2,000) if:

  • You are disabled as defined by the LTD Plan,
  • You continue to be disabled beyond the 26 weeks of your short-term disability,
  • You provide documentation to UNUM and Cummins as requested, and
  • UNUM confirms that you are eligible for LTD payments.

LTD benefits are coordinated with Social Security or Workers’ Compensation or other related payments, so that the total amount of benefits you receive while disabled is 60% of your base pay (up to a monthly maximum of $2,000).

LTD benefits are fully insured by UNUM.

Long-Term Disability (Applicable to employees hired after June 19, 1990)

For disability claims effective after May 3, 2016, Long Term Disability (LTD) benefits will begin after the 26 week Short Term Disability (STD) benefit has been exhausted. Employees who continue to maintain their disability status beyond 26 weeks will receive 60% of base wages until age 65. Medical, dental, and life insurance will continue to be available for up to two (2) years from the beginning of the LTD benefit, as long as disability eligibility is maintained.

What is the definition of Long Term disability (LTD)?
Long-Term Disability pays a portion of your pay while you are totally disabled and continue to be away from work for an extended period. Cummins pays the full cost of your Long-Term Disability coverage.

ou are eligible for LTD benefits when Unum determines that:

  • You are limited from performing the material and substantial duties of your regular occupation due to your sickness or injury; and
  • You have a 20% or more loss in your indexed monthly earnings due to the same sickness or injury.

After 24 months of payments, you are disabled when Unum determines that due to the same sickness or injury, you are unable to perform the duties of any gainful occupation for which you are reasonably fitted by education, training or experience. You must be under the regular care of a physician to be considered disabled. The loss of a professional or occupational license or certification does not constitute disability.

We may require you to be examined by a physician, other medical practitioner and/or vocational expert of our choice. Unum will pay for this examination. We can require an examination as often as it is reasonable to do so. We may also require you to be interviewed by an authorized Unum Representative.

When do you notify Unum of a claim?
We encourage you to notify Unum of your claim as soon as possible, so that a claim decision can be made in a timely manner. Written notice of a claim should be sent within 30 days after the date your disability begins. However, you must send Unum written proof of your claim no later than 90 days after your elimination period. If it is not possible to give proof within 90 days, it must be given no later than 1 year after the time proof is otherwise required except in the absence of legal capacity.

What if I have an accident or extended illness?

Be sure to notify your manager and UNUM at 1-866-229-4885 so that disability benefits may begin.
